The Medication Therapy Management (MTM) Pharmacy Student Patient Engagement Coordinator will assist MTM Pharmacists to support our mission of delivering safe, effective, and affordable medication therapy to our target population. The MTM Pharmacy Student Patient Engagement Coordinator assists pharmacists in providing outstanding patient care and customer service through targeted medication reviews, intervention follow-up, and scheduling medication reviews. In addition, they will perform technical and clerical duties which do not require the professional clinical judgment of the pharmacist.


Accountabilities:

Scheduling Activities

  • Contact patients to introduce and explain the benefits of the MTM program

  • Schedule appointments for the MTM Pharmacist(s) to complete Comprehensive Medication Reviews, Disease State Medication Reviews, or Targeted Interventions

    Patient Care Activities

    • Deliver Targeted Interventions to assess appropriateness of medications, to optimize medication therapy and to improve clinical outcomes

    • Conduct patient-centered Adherence Monitoring MTM services to help patients identify barriers with adherence and develop solutions to overcome those barriers

    • Assist with managing medication costs through cost-effective interventions which may include, but are not limited to formulary and therapeutic interchanges, pill splitting opportunities, and assessing unnecessary and/or duplicate medication therapies

    • Receive and respond to incoming patient calls; take appropriate action based off protocols and use critical thinking skills to escalate calls to appropriate parties when needed

    • Accurately document all discussions with patients and schedule follow-up as needed

    • Accurately update patient demographic, allergy, and medication information in the MTM platform as necessary

    • Obtain correct/updated health care provider contact information

      General

      • Provide exemplary internal and external customer service through prompt, professional, and polite communication

      • Perform auxiliary functions essential to the efficient operation of the department

      • Meet productivity and performance expectations set forth by manager

        Qualifications:

        Required –

        • Fluent in Vietnamese (ability to write and speak at a healthcare competency level)

        • Active pharmacy intern or pharmacy technician license in the state of residence, where required

      • Extensive knowledge of medications and medical terminology, including: ability to use and pronounce common medical terms, pharmacy terms, and medication names correctly; knowledge of pharmaceutical therapeutic categories and indications

    • Ability to receive and critically evaluate information from both oral and written sources, including pharmaceutical references, formularies, disease state guidelines, and peer-reviewed publications

  • Understanding of the pharmacist’s clinical role in patient care

Ability to follow step-by-step instructions and enter data accurately into MTM database with close attention to detail

Ability to provide excellent customer service with a can-do attitude

Proven record of excellent attendance and punctuality

Proven ability to work independently in a collaborative environment

Preferred –

Enrollment in Second or Third Year of Pharmacy School Curriculum

Outpatient pharmacy technician experience

Customer service experience

High School diploma, GED or equivalent, or equivalent work experience, preferred

Medical background or experience in a health care setting

Experience with MTM Platforms, electronic health records, and/or scheduling software

Self-management Skills – Self-motivation, organization, and attention to detail

Knowledge of the Law – Understanding of basic compliance measures for the CMS programs, and ensure privacy and confidentiality as required by HIPAA, company and department guidelines

Computer/Technology Competency- Proficiency in use of Microsoft Word, Excel, and Outlook, with strong data entry skills and ability to multitask using multiple databases/applications

Remote Telecommuter, United States


Availability:

Must provide a minimum of 10 hours of availability per work week during the hours of 8 AM to 7 PM CST Monday through Friday, and 9 AM to 5:00 PM CST on Saturdays

Two Saturdays per month (minimum 4-hour shift)

Consideration given to candidate’s pharmacy school schedule

Anticipated Pay: $15/HR
